Simple columnar epithelium consists of a single layer of tall, column-shaped cells with basally located, elongated nuclei. It is specialized for absorption and secretion, and is found lining the GI tract from stomach to anus, the gallbladder, and parts of the female reproductive tract.

Simple columnar epithelium is composed of a single layer of tall, rectangular cells whose height exceeds their width. Nuclei are typically oval and located near the base of the cell. This epithelium type is highly specialized for absorption (small intestine) and secretion (stomach, glands), often featuring surface modifications such as microvilli forming a brush border to increase surface area for nutrient absorption, or cilia in regions like the fallopian tubes and uterus to aid in movement of materials. Goblet cells, which secrete mucus, are frequently interspersed within this epithelium in the GI tract and respiratory regions lined by pseudostratified columnar epithelium. Clinically, this epithelium type is relevant in understanding conditions such as Barrett's esophagus, where normal stratified squamous epithelium of the esophagus undergoes metaplasia to simple columnar epithelium with goblet cells due to chronic acid reflux (GERD), increasing risk for esophageal adenocarcinoma. Additionally, in the stomach, simple columnar epithelium lines the gastric pits and glands, secreting mucus, acid, and enzymes, making it relevant to peptic ulcer disease pathology. In the intestines, loss of the columnar brush border (as in celiac disease) leads to malabsorption. Understanding the structure-function relationship of this epithelium is essential for interpreting histology slides and correlating with pathologies involving absorption, secretion, or metaplastic changes.
